                     Tacrolimus, also known as FK506, is a fermentation product isolated from Streptomyces. It is a macrolide antibiotic and a powerful new immunosuppressant, mainly by inhibiting the production of interleukin-2 (IL-2). Release, comprehensively inhibit the effect of T lymphocytes, 100 times stronger than cyclosporine (CsA). In recent years, as a first-line drug for liver and kidney transplantation, it has been marketed in 14 countries including Japan and the United States. Clinical experiments show that it has a good effect in the transplantation of heart, lung, intestine and bone marrow. At the same time, FK506 also plays an active role in the treatment of autoimmune diseases such as atopic dermatitis (AD), systemic lupus erythematosus (SLE), and autoimmune eye disease.

Pharmacological action

                        At the molecular level, the effect of tacrolimus appears to be by binding to a cellular protein (FKBP12), which accumulates in cells for its effect. The FKBP12-tacrolimus complex specifically binds and inhibits calcinurin, which inhibits calcium-dependent signaling pathways in T cells, thereby preventing the transcription of discontinuous lymphokine genes. This drug is a highly immunosuppressive drug, and its activity has been confirmed in vitro and in vivo experiments. This drug inhibits the formation of cytotoxic lymphocytes that are responsible for the main effect of transplant rejection. The drug inhibits the activation of T cells and the proliferation of T helper cells dependent on B cells. It also inhibits the production of lymphokines such as interleukin-2, interleukin-3 and gamma-interferon and the expression of interleukin-2 receptors. At the molecular level, the drug's effects appear to result from binding to a cellular protein (FKBP), which also causes the compound to accumulate in cells. In vivo, the drug has shown efficacy in liver and kidney transplantation.
